Brief review on COVID-19 pandemic in Nepal

  • The first case in Nepal was confirmed on 23 January 2020 
  • The second case was confirmed on 23 March 2020 in Kathmandu. 
  • The first case of local transmission was confirmed on 4 April in Kailali District. 
  • The first death occurred on 14 May
  • A total of 5760 cases have been confirmed as of Wednesday, June 14, 2020.
  • Since late January, 1,33,377 RT-PCR tests have been performed in 20 laboratories across the country, 
  • A country-wide lockdown came into effect on 24 March 2020, and is ended on June 14, 2020.

Required Features of Viral Transport Medium (VTM)

  • VTM Sustains viability for culture and Nucleic Acid Amplification Test (NAAT) of viruses, chlamydiae, mycoplasmas and ureaplasmas in a single formulation by “preserving” and “stabilizing” them,
  • Prevents specimen drying
  • Offers flexibility in transport temperature ranges–room temperature or refrigerated.
  • Suppresses bacterial and fungal contamination by incorporating antibiotics in the medium.
  • Preserves viruses and chlamydiae for long-term frozen storage

Specimens from humans :-

A suitable VTM for use in collecting throat and nasal swabs from human patients is prepared as follows: –

Add 10g veal infusion broth and

2g bovine albumin fraction V to sterile distilled water (to 400 ml). –

Add 0.8 ml Gentamicin sulfate solution (50 mg/ml) and

3.2 ml Amphotericin B (250 µg/ml) – Sterilize by filtration.

  • Amphotericin B is an antifungal medication .
  • Gentamicin is a broad spectrum antibacterial

Note:

  • Normal saline (NS) solution should not be used as a VTM.
  • Adding BSA ( bovine serum albumin) and antibiotics to NS changes the pH and this will destroy viruses.
