Sevelomer: Uses, Mechanism, Side Effects & Role in Kidney Patients

Sevelomer is a widely prescribed drug for patients suffering from chronic kidney disease (CKD). It plays an important role in controlling high phosphate levels in the blood, which is a common complication in kidney patients.

What is Sevelomer?

Sevelomer is a non-absorbed phosphate binder used to reduce elevated serum phosphate levels (hyperphosphatemia). It is available in two salt forms:

  • Sevelomer hydrochloride
  • Sevelomer carbonate

Both forms work similarly, but the carbonate form is often preferred because it avoids the risk of acidosis.

Uses of Sevelomer

  • Treatment of hyperphosphatemia in patients with chronic kidney disease (CKD) on dialysis.
  • Helps prevent complications like bone disease, vascular calcification, and cardiovascular problems caused by excess phosphate.
  • Used as an alternative to calcium-based phosphate binders, especially in patients with high calcium levels or risk of calcification.

Mechanism of Action

  • Sevelomer binds to phosphate present in food within the gastrointestinal tract.
  • This binding forms an insoluble complex that is not absorbed into the bloodstream.
  • As a result, phosphate is excreted in feces, lowering serum phosphate levels.
  • Unlike calcium-based binders, Sevelomer does not increase calcium levels, reducing the risk of calcification.

Side Effects of Sevelomer

Common side effects include:

  • Nausea
  • Vomiting
  • Constipation
  • Diarrhea
  • Abdominal pain

Less common but important:

  • Risk of bowel obstruction or perforation in severe cases.

Why is Sevelomer Used in Kidney Patients?

Kidneys normally help maintain phosphate balance. In chronic kidney disease, reduced kidney function leads to:

  • Phosphate retention
  • Hyperphosphatemia → which causes bone weakness (renal osteodystrophy) and vascular calcification.

Sevelomer helps by binding excess phosphate in the gut, thereby:

  • Lowering serum phosphate levels
  • Protecting bones
  • Reducing the risk of cardiovascular complications
  • Providing an option for patients who cannot tolerate calcium-based binders
