Sildenafil(Viagra): Mechanism of Action, Uses, Contraindications, and Side Effects

Introduction

Sildenafil(Viagra) is a well-known medication primarily used for the treatment of erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). It belongs to the class of phosphodiesterase type 5 (PDE5) inhibitors, which help in vasodilation and improved blood flow. This article explores how sildenafil works, its uses, contraindications, and potential side effects.

Mechanism of Action

Sildenafil(Viagra) works by inhibiting the enzyme phosphodiesterase type 5 (PDE5), which is responsible for the breakdown of cyclic guanosine monophosphate (cGMP). In normal physiology, sexual stimulation leads to the release of nitric oxide (NO), which increases cGMP levels, causing smooth muscle relaxation in the corpus cavernosum of the penis and allowing increased blood flow, leading to an erection. By preventing cGMP degradation, sildenafil enhances and prolongs this process.

In pulmonary arterial hypertension, Sildenafil(Viagra) reduces pulmonary vascular resistance, leading to improved oxygenation and reduced workload on the heart.

Uses of Sildenafil(Viagra)

  1. Erectile Dysfunction (ED) – The primary use of Sildenafil(Viagra) is to help men achieve and maintain an erection.
  2. Pulmonary Arterial Hypertension (PAH) – Sildenafil(Viagra) helps reduce blood pressure in the lungs, improving exercise capacity.
  3. Raynaud’s Phenomenon – Off-label use in treating vasospastic disorders.
  4. High-Altitude Pulmonary Edema (HAPE) – Occasionally used in high-altitude sickness.

Contraindications Sildenafil(Viagra)

Sildenafil(Viagra) is not suitable for everyone. It is contraindicated in the following cases:

  1. Patients using nitrates – Concurrent use of nitrates (e.g., nitroglycerin, isosorbide dinitrate) can cause severe hypotension.
  2. Severe cardiovascular conditions – Patients with recent heart attack, stroke, or unstable angina should avoid sildenafil.
  3. Hypotension – Individuals with low blood pressure (<90/50 mmHg) should not take sildenafil.
  4. Severe liver or kidney disease – Impaired metabolism can lead to drug accumulation and increased risk of adverse effects.
  5. Retinitis Pigmentosa – A rare genetic disorder affecting vision, where PDE5 inhibitors can worsen retinal damage.
  6. Allergy to Sildenafil – Patients with hypersensitivity to sildenafil or its components should avoid it.

Side Effects of Sildenafil(Viagra)

Like all medications, Sildenafil(Viagra) has potential side effects, including:

  1. Common Side Effects:
    • Headache
    • Flushing
    • Nasal congestion
    • Indigestion
    • Dizziness
    • Vision disturbances (blurred vision, blue-tinted vision)
  2. Serious Side Effects:
    • Priapism (prolonged erection lasting more than 4 hours)
    • Severe hypotension
    • Sudden hearing or vision loss
    • Chest pain or irregular heartbeat
    • Allergic reactions (rash, swelling, difficulty breathing)

While it offers significant benefits, it must be used with caution, particularly in individuals with pre-existing cardiovascular conditions or those taking nitrates.
